NCLEX-RN Alternate Format Questions: Fill-in-the-Blank

NCLEX-RN® Fill-in-the-Blank questions ask you to fill in the blank with a number based on a calculation. Like select all that applyhot spot, and drag-and-drop questions, these types of questions are called alternate format questions and are part of the test pool of questions for the NCLEX-RN exam.

To answer this question you need to know that intake includes what the client drinks along with the amount of dialysate that is retained after the one-hour exchange of solution.
First, convert cups into ounces. One cup of fluid = 8 oz. Then convert ounces into milliliters. One ounce = 30 milliliters.
The client’s intake is:
8 oz. apple juice = 240 mL
2 cups = 16 oz. water = 480 mL
100 mL = retained dialysate
